Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all estimated costs before treatment is administered. Insurance coverage is only an estimation. Guarantor is responsible for all treatment not covered by insurance.

Payment is due at the time of service.

If special arrangements are needed, please talk to our Account Specialist prior to receiving service.

We will submit all insurance claims for you. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for any unpaid claims within 30 days.
